Tourette’s and anxiety: the hidden driver of tics

Anxiety and Tourette’s often coexist in a symbiotic and exhausting relationship. If we imagine tics as an electric shock, anxiety is the generator that cranks up the voltage. It is not uncommon for a person with Tourette’s to also suffer from generalised or social anxiety disorder. This is no coincidence, but the result of a nervous system that is constantly in a state of hyper-arousal.

Living with the feeling that a tic could erupt at any moment creates an underlying tension that never leaves the body. This ‘waiting’ consumes immense mental resources and ends up fuelling precisely what one would like to avoid. Understanding how to deactivate this hidden trigger is essential for improving quality of life and reducing the impact of the syndrome in daily life.

The link between anxiety and tics

At a biochemical level, anxiety activates the hypothalamic-pituitary-adrenal axis, which releases cortisol and adrenaline. These substances prepare the body for action (fight or flight), increasing the excitability of motor neurons. In a Tourette’s brain, this excitation immediately translates into a burst of more frequent and intense tics. Anxiety acts like a fertiliser: it does not create the seeds of the tic, but makes them grow and multiply rapidly.

Why tension increases symptoms

Physical tension is the language through which anxiety communicates with the muscles. When you are anxious, your muscles are already contracted and ready to spring into action. This physical rigidity lowers the tolerance threshold for the premonitory stimulus (that sensation of itching or tension that precedes the tic). Consequently, the brain feels the need to release the tension much more frequently than usual. The more rigid the body is, the more the tic becomes necessary to regain, even for just a second, a sense of relief.

Anticipatory anxiety: the fear of the tic

Anticipatory anxiety is the worry that arises even before anything happens. In this case, it is the fear that a tic will appear at an inappropriate moment, such as during an exam or an important appointment. This fear creates a state of extreme vigilance which, paradoxically, triggers the very tic that is feared. It is a self-fulfilling prophecy: you worry about having a tic, the worry increases anxiety, and the anxiety sets off the tic.

The cycle between stress and movement

A vicious circle is often created that is difficult to break. The stress of daily life increases tics; visible tics cause embarrassment and social stress; embarrassment increases anxiety, which in turn generates more tics. In this cycle, the person feels trapped in a body that betrays their intentions. Breaking this loop requires stopping the fight against the movement and starting to work on managing the stress that fuels it.

How to lower anxiety levels

The most scientifically sound strategy is to target the autonomic nervous system. You cannot simply ‘decide’ not to be anxious, but you can teach your body to relax. Maintaining a predictable routine reduces uncertainty, which is the brain’s primary source of anxiety. Limiting the use of stimulants such as caffeine can also help prevent overloading an already hyper-excitable system. The aim is not to eliminate anxiety, but to lower its volume so that it does not become the dominant noise.

Techniques that really help

Diaphragmatic breathing techniques are powerful tools because they send a physical signal of calm to the brain. Biofeedback and cognitive behavioural therapy (CBT) are also very effective for learning to manage catastrophic thoughts linked to tics. The practice of radical acceptance (Mindfulness) teaches you to observe the tic without judging it, removing the emotional charge that makes it explosive. Remember: the less importance you attach to anxiety, the less fuel there will be for your tics.
