Tourette’s and stress
In this chapter, we explore how stress is not merely an emotion, but a genuine biological modulator of the syndrome. If Tourette’s is the engine, stress is the load that causes it to overheat. Understanding how the nervous system reacts to pressure enables us to stop simply enduring the tics and start managing the environment around them.
The relationship between stress and Tourette’s is two-way: stress increases tics, and the tics themselves, due to the physical and social fatigue they entail, generate further stress. Breaking this cycle requires a careful analysis of how our bodies process fatigue and external pressure. It is not a question of eliminating stress — an impossible task in modern life — but of learning to balance the load with effective recovery strategies.
Acute stress vs chronic stress
Acute stress is the immediate reaction to an event, such as an exam or a sudden appointment. At such times, tics can erupt violently due to the surge in adrenaline. Chronic stress, on the other hand, is that constant tension that lasts for weeks or months. This type of stress is more dangerous because it wears down the brain’s inhibitory circuits, making them less able to filter movements. Whilst acute stress creates spikes, chronic stress persistently raises the ‘baseline frequency’ of tics.
Why the body reacts this way
From a scientific perspective, stress activates the sympathetic nervous system, which is programmed for survival. In this state, the brain prioritises rapid action over calm reflection. For those with Tourette’s, this activation results in a lowering of the threshold of the basal ganglia, our motor filters. The body reacts by discharging energy through tics because the nervous system is too excited to maintain control. It is a physical response to internal chemistry running at full speed.
The most critical times of day
There are periods when accumulated stress makes itself felt most strongly. Often, returning home after a day at school or work is the most critical moment. The brain, having ‘held out’ for hours in public, lets go and releases all the stress in the form of tics. Waking up can also be challenging, as the body prepares for the day by raising cortisol levels. Identifying these times allows you to schedule the moments of decompression the system needs.
The role of recovery
Recovery is not just rest, but a genuine biological necessity to ‘reset’ the nervous system. Without quality sleep and genuine breaks, the brain cannot clear stress neurotransmitters. Recovery allows the prefrontal circuits to recharge and resume functioning as effective filters. For those with Tourette’s, sleeping well is not a luxury, but the primary form of natural therapy. A well-rested body has a much higher tolerance threshold for tics than an exhausted one.
Habits that worsen stress
Certain daily behaviours can unintentionally fuel the system’s tension. Excessive use of screens and social media keeps the brain in a state of constant overstimulation. A diet high in sugar and caffeine also acts as a chemical stressor for motor neurons. A tendency towards perfectionism or the attempt to hide tics at all costs are other sources of self-inflicted stress. It is often the small habits that create that ‘background noise’ which makes tics harder to manage.
How to build balance
Building balance means learning to read your body’s warning signs before an outburst. Taking micro-breaks throughout the day helps prevent tension from building up to breaking point. Moderate physical activity is an excellent way to burn off excess adrenaline in a controlled manner. Engaging in hobbies that fully absorb your attention can give the brain those moments of ‘flow’ where tics disappear. Balance is not the absence of tics, but the ability to live a full life whilst keeping internal pressure low.
