Tourette’s and impulsivity: why you act before you think
Impulsivity is often described as the engine of Tourette’s, running without anyone pressing the accelerator. It is not a lack of thought or a difficult personality, but a feature of the brain’s operating system. In people with Tourette’s, the time between impulse and action is drastically reduced. It is as if the brain skips the review phase and goes straight to execution, leaving little room to assess the consequences.
Neurologically, this phenomenon reflects a difficulty in response inhibition processes. The brain receives thousands of stimuli per second and must decide which to discard and which to act upon. In Tourette’s, the filter that should block unnecessary responses is less effective than usual. Understanding this mechanism helps transform frustration into conscious management of one’s mental and physical reflexes.
What impulsivity really is
Impulsivity is not an emotion, but a cognitive function linked to the regulation of behaviour. Scientifically, it is divided into motor impulsivity (acting without thinking) and decision-making impulsivity (choosing without evaluating). In Tourette’s, both are often present due to accelerated communication between the frontal lobes and the basal ganglia. It is the nervous system’s innate tendency to prefer immediate action over waiting or planning.
Why the brain ‘anticipates’ actions
The brain of someone with Tourette’s lives in a state of constant ‘motor readiness’. The circuits that prepare movement are hyperexcitable and ready to release energy at the slightest input. This early activation causes the action to start before the centres of logical control can give the go-ahead. It is a purely electrical phenomenon: the motor signal is simply faster than the inhibitory signal that should stop it.
The difference between choice and reaction
It is essential to distinguish between a deliberate act and an impulsive reaction linked to the syndrome. A choice involves weighing up the pros and cons, whereas an impulsive reaction is automatic and almost reflexive. Many people with Tourette’s describe impulsivity as a momentary ‘seizure’ of the will. There is no intention behind the impulsive act, but only a biological command that has bypassed consciousness to manifest itself in the real world.
Social impulsivity: saying or doing too much
Impulsivity can also manifest itself in speech and interactions with others. You might find yourself interrupting someone, responding too quickly or speaking an uncomfortable truth without filtering it. This happens because ‘social brakes’ reside in the same areas of the brain that control motor tics. When the system is overloaded, the ability to monitor one’s verbal behaviour diminishes. It is not rudeness, but a technical difficulty in calibrating communication in real time.
The consequences in daily life
Living with high impulsivity can lead to frequent misunderstandings and a sense of mental exhaustion. People make small, distracted mistakes or take hasty decisions that then require effort to rectify. Socially, one may feel that one is ‘too intense’ or difficult for others to keep up with. These consequences often fuel a sense of anxiety which, paradoxically, further increases the system’s impulsivity.
Strategies for slowing down
Although you cannot change your biology, you can practise strategies to create a ‘buffer’ between impulse and action. The most effective technique is ‘deliberate delay’: counting to three before responding or acting. Meditation and mindfulness exercises also help to strengthen the inhibitory circuits of the prefrontal cortex. Creating an environment with fewer chaotic stimuli prevents the brain from becoming overloaded. The key is to learn to recognise your triggers so you can anticipate the impulse before it turns into action.
